Please select this test when you are booking your Stage One FACT test for the first time. You must select it on the same date as your Stage One. 

  • Please also choose this option if you are renewing your Stage One and want to renew your Stage Two Hearing ON THE SAME DAY.
  • If you took your Stage One in OTTAWA - you may select this test for your Hearing Assessment.
  • On your test date, once you have completed your Stage One test you will proceed to the Hearing Assessment. There is no assigned time for your Hearing Assessment, it is first come, first served.
  • Your paid receipt/confirmation is automatically generated through this website and will be emailed to you for your records. Incorrect bookings will result in an automatic refund, less a $ 50 plus HST administration fee.
  • There is currently NO re-scheduling. Please do not book your appointment until you are sure of your personal and professional schedules.

Cancellation Policy: If you wish to cancel your assessment and it is five business days or greater from your assessment date/time, you must request a cancellation through your online account at www.ofai.ca  by selecting the "Cancel" button next to the order/assessment you are cancelling. You will be refunded, less a $50.00 plus HST administration fee.
If you must cancel your assessment within five business days of your assessment date/time, you will not be refunded.  Please advise the administration office you will not be attending your assessment at  info@ofai.ca. It is the  candidate's responsibility to abide by the cancellation policy on each assessment.

Re-Test Policy: All OFAI failed assessments are subject to our re-test policy.  The re-test policy identifies the wait times candidates must follow before coming back to re-test. The wait times are put in place to allow each candidate sufficient time to prepare (and in some cases, train) in order to be successful in their assessments.

The following identifies the OFAI re-test policy wait times:
1st Failure - 15 Days
2nd Failure - 30 Days
Any subsequent failure will be subject to a 30-day re-test policy.

Arrival times:
Stage One: 15 minutes prior to scheduled time
Stage Two Hearing: 15 minutes prior to scheduled time
Stage Two Encapsulated Treadmill:  30 minutes prior to scheduled time
Stage Three & FPAT:  30 minutes prior to scheduled time

If you are late for your appointment, you will be required to wait.  If the evaluation staff are unable to accommodate you in the schedule,then you will be required to rebook at your own expense.
